Same-Day Crowns Aren’t All Created Equal

In our modern world, we’ve come to expect that fast usually means a compromise in quality. We accept that a fast-food meal won't be a culinary masterpiece, and a quick fix for a car might only get us a few more miles down the road. For a long time, dentistry was the same. If you wanted a dental crown that actually lasted, you had to wear a plastic temporary restoration for two to three weeks and wait for a dental lab to send back the permanent version.

Then came the same day dental crowns. Suddenly, you could walk into the dentist's office with a broken tooth and walk out a few hours later with a permanent crown. It sounds like magic, and for many in San Diego, it’s a massive convenience. But as a prosthodontist, I’ve seen that this speed has created a new problem. Many people now believe that because the technology is fast, the work is simple. They assume that every new crown is the same because the software is doing the heavy lifting.

I want to be very clear: A fast crown is only a value if it is also a permanent, high-quality solution. At San Diego Dental Center, we use technology to save you time, but we never let the technology rush our clinical judgment. We take the time to make sure it is done right the first time.

The Machine is the Tool, Not the Doctor

The technology we use, commonly known as computer-aided design (CAD), is remarkable. It uses a high-definition camera to take digital impressions of your mouth, and a milling machine carves your tooth out of a solid block of ceramic. It is precise, clean, and avoids the messy putty required for a traditional crown.

However, the machine has default settings. If a dentist simply clicks print and accepts whatever the software suggests, the result is often a crown that fits okay, but not perfectly. As a specialist, I spend a significant amount of time overriding the computer. I manually adjust the margins, the tiny area where the new crown meets your natural enamel, and the occlusion, which is how the tooth hits the opposing teeth when you chew.

Technology can give us speed, but it cannot give us wisdom. The machine doesn't know your history, your biting and chewing forces, or how your jaw moves when you’re stressed. I use the digital scan as a blueprint, but I do the final engineering myself. If a crown isn't designed to last twenty years, it doesn't matter if it only took one visit to make.

Why the Material and Design Matter

One reason same-day dental crowns aren't created equal is the material itself. In some offices, they use a one-size-fits-all ceramic block because it’s easy to mill. But your front and back teeth serve different purposes. Your out-of-sight molars bear the brunt of chewing forces, requiring materials that are extremely durable. Meanwhile, your front teeth need a custom crown that matches the translucency of your natural teeth.

In the past, metal crowns or porcelain fused to metal (PFM) were the standard for strength. While metal crowns rarely chip, many patients today dislike the metallic color and potential for an allergic reaction or metal allergies. Our all-ceramic options offer the strength of metal crowns without the aesthetic drawback.

We also take the extra step of characterizing the crown. Most day dental restorations come out of the machine looking like a single, flat color, which some call a chiclet tooth. We hand-stain and glaze every restoration to match the unique shades of your tooth enamel. We want your crown to disappear into your smile, not stand out as a piece of dental work.

The Hidden Risks of the Rushed Fit

When a dental crown procedure is rushed, the biggest risk is a poor fit at the gumline. If there is even a microscopic gap between the crown and the damaged tooth, bacteria will find it. This leads to decay under the crown, often affecting decayed teeth that were already vulnerable.

Unlike the traditional ones that require two appointments, the same-day process eliminates the need for temporary crowns. This is a major benefit, as temporary restoration pieces are often held in with temporary cement and can lead to a loose crown or cause pain if they fall off. However, skipping the second visit only works if the final crown is perfectly sealed. We take the time to ensure a hermetic seal that protects your gums and the entire tooth.

I’ve seen many patients come to me to replace a same-day crown they received elsewhere just a year or two prior. The issue is almost always a lack of precision during the bonding phase. This isn’t just a large filling material or a temporary fix. We chemically fuse the replacement tooth to the structure. That takes a few extra minutes of focus, but it’s the difference between a repair and a restoration.

Is a Same-Day Crown Right for Every Case?

While day crowns are a fantastic procedure for most, they aren't a universal solution. Honesty is a core part of our practice and a key standard of the American Dental Association. If you have a cracked tooth that extends below the gumline, a weak tooth that needs a root canal, or a dental implant that requires a specific type of dental lab fabrication, we will tell you.

Sometimes, a traditional crown is the better treatment for complex cases involving other teeth or specific types of crowns. We would rather ask you to come back for a second visit and get a perfect result than rush a one-visit result that doesn't meet my standards for a healthy smile.

Caring for Your New Restoration

Once your custom crown is placed, proper care is essential for oral health. Even though the crown itself cannot decay, the natural enamel underneath it can. We always recommend:

  • Using an antibacterial mouthwash to keep the gums healthy
  • Avoiding chewy foods or chewy foods like taffy in the first 24 hours while the bond fully sets
  • Brushing and flossing as you would with your natural teeth to protect against future decayed teeth

Done Right the First Time

Our goal in our office is to make sure you never have to think about this tooth again once you leave. The convenience of a one-visit appointment is a wonderful benefit, but the true value is the peace of mind that comes with a permanent solution.

Look for the prosthodontist who is willing to slow down and make sure the fast option is also the right one.
